Sealift Claims Handling Rules and Forms
The handling of disputes relating to the Sealift program is governed by the Sealift Claims Handling Rules, which set out the procedures and timing for processing a claim by a Shipper against their Carrier (shipping company). Please be aware that failure to comply with certain steps and time deadlines may result in your claim being barred. Timing details are set out in the Sealift Claims Handling Rules.
The forms to use in processing a claim under the Sealift program are available for download:
Form 2- Acknowledgment (

PDF) (This form must be used by the Carrier to acknowledge receipt of the Form 1-Notice of Claim and to seek any additional information required to process the claim.)
Form 3- Notice of Decision (

PDF) (This form must be used by the Carrier to notify the Shipper of their decision on the merits of the claim.)
Form 4- Application for Arbitration (

PDF) (This form must be used by a Shipper to commence arbitration proceedings against a Shipper and set out the details of the Shipper’s claim.)
Form 6- Shipper's Reply (

PDF) (This form may be used by the Shipper, if needed, to reply to any new arguments raised by the Carrier in the Carrier’s Response.)
Form 7- Carrier's Reply (

PDF) (This form may be used by the Carrier, if needed, to reply to any new arguments raised by the Shipper in the Shipper’s reply.)
